Dear Patient,

Welcome to our practice. Thank you for choosing to schedule your appointment with Dr. Terrence W. Coleman. Below you will find a list of questions frequently asked by new patients. We hope you will find this list of questions and answers helpful. If you have any other questions, please call our office at 816-429-6057.  Our office hours are Monday through Thursday, 9:00am until 4:30pm.  We are closed daily for lunch between 12:00pm and 1:00pm.  We are closed on Friday.

  1. Do I need to bring a referral?
    Dr. Coleman requires a referral from your Primary Care Physician, regardless of your insurance requirements.  Please contact your primary care physician's office to begin the referral process.
  2. When do I pay my copay?
    You are required to pay your copay in full at time of service. You have a contract with your insurance company to pay your copay. We have a contract with your insurance company to collect your copay. You may pay with a credit card, debit card, personal check or cash.  We can not bill you for your copay amount due at the time of service. If you cannot pay at of your appointment, please call us to reschedule.
  3. What will I need for my appointment?
    You will need two forms of identification (one of which is a picture ID), and you will also need a valid insurance card unless you are a self-pay patient. Also, please fill out the patient history form.  Be sure to include an accurate list of all medications you take, even over-the-counter medications and supplemental medications (to include herbal supplements/medications). Please note dosage and scheduled times of your medications on the list.  Please return your completed forms to our office as soon as possible to help us expedite your check-in time.
  4. Can I take a cab home after a procedure?
    NO, you may not take a cab home after a procedure. Your driver will be required to check in with you at the time of your procedure. It is preferred that your driver remain on campus during your procedure.  If you are not having a procedure and are coming for an office visit, you will not need a driver.
  5. How soon can I obtain results from tests performed at the time of my appointment?
    We will explain any test results as soon as Dr. Coleman has received the final report from the laboratory, pathology or x-ray department. This usually takes 7-10 working days. Please be sure to provide our office with a daytime phone number or cell phone number.  If you have a call blocking feature on your phone, it is your responsibility to contact our office to obtain test results.
